Criteria
The grant applicant must meet the following criteria in order to be considered. Information Requested • A completed Application form How to submit your request: All requests must be submitted as a Word document. Type the application answers directly in the provided application document. Telephone requests are not accepted. All areas of the application must be completed in order for the application to be accepted. Cassie Beverly Grants are distributed quarterly, no earlier than two weeks after the Foundation quarterly meeting date. No grants will be issued before a meeting date. Please send your request at least two weeks prior to the meeting date. Foundation quarterly meetings are scheduled as follows: Quarterly Foundation Meeting Date: All Grants Due By:
Overall Instructions: There are eight sections in the application: Instructions for Part A: Application Information 1. Enter your organization’s Federal Employer ID Number, also known as the Taxpayer ID Number. This is the nine-digit number that was assigned by the IRS. Do not use a Social Security Number. This number is used to confirm your federal tax-exempt status and to report any payments to the IRS. If you don’t know your FEIN number visit www.sos.state.ia.us/corp/corp search.asp 2. Enter the name of the organization that will be conducting the grant activity. 3. Enter the name and contact information of the person primarily responsible for managing the grant activity. Instructions for Part B: Project Information 4. If this is a continuation of a similar project, attach your most recent evaluation results. 5. Support may not be requested sooner than two weeks after the quarterly meeting date when your project will be reviewed. Please see the timeline in the How to Submit Your Request section above. 6. Enter the name or title of the grant activity. Simple, descriptive titles are best. Give a brief, 50-word or less description of your project or a description of how the grant will be used. 7. Include the amount requested only. 8. Identify the exact expenses the Regency Foundation grant will support. Be as specific as possible.
Instructions for Part C: Application Narrative 9. Describe your organization (type of organization, mission and purpose, primary service area, amount of people served, number of members, etc). Use one paragraph for this information. 10. Use approximately one half page for this section. Describe how your organization will use the grant to enrich the lives of Iowans. Who is the target audience? Who will benefit from this project? Why is it important this project happen or that the grant money is awarded? 11. Use approximately one-half page for this section. Describe the planning and processes that will help you achieve the goals you have set. What are your goals for this project? How will you market and promote this project? How will you know if the project has been successful? How can the Regency Foundation be assured the grant has been useful to Iowans? Instructions for Part D: Support Materials and Work Samples Form Support materials and work samples help reviewers determine the overall strength and quality of the project/organization and the qualifications of the applicant to successfully complete the grant activity as described in the application. The quality and appropriateness of the support materials and work samples are contributing factors in the review process. Limit the number of support items. Submit only few items that are specifically related to and support key points of the application narrative. High quality work samples are particularly important. Letters of support are also accepted as support material. All support materials/items given for review must be copied on 8.5 x 11 paper. Please do not submit original artwork. Instructions for Part E: Application Checklist • Complete the Application Checklist and submit with your entire application package.
Instructions for Part F: Service Contract If your application is funded, this Service Contract becomes effective upon issuance of a Grant Award Letter to the Applicant by the Regency Foundation and upon the Regency Foundation’s president signing the Service Contract. The Applicant’s legal name and address used in Part A must also be used in this section. An individual who has the authority to legally obligate the Applicant to the conditions of the grant program and Service Contract must sign the Service Contract.
